Forms are provided to record the bowlers participating in a match Line-up Form, and to record the match results Results Form, and an additional form with instructions on properly completing the results form Results Form with Instructions.  The completed match form is to be FAXed to the commissioner and to The Repository within one hour of the completion of the match so that they may be published.

As an alternative, an Excel version of the match recap form has been developed to enable the match results to be recorded on a PC and then submitted.

A new form has been created in Excel with a scoring sheet background.  The spreadsheet uses a trick to print a picture of the background in the header of the document.  This will allow typing in the form as long as the magnifications are not altered.
